A Multicenter, Randomized, Double-Blind Comparison of Intravenous Iron Supplementation to both Enteral Iron Supplementation and Placebo for the Treatment of Anemia of Traumatic Critical Illness

Trauma patients in the ICU are almost always anemic, putting them at risk for infection, impaired wound healing, and death. Current therapies for ICU anemia remain unsatisfactory, presenting additional risks, poor outcomes, and high cost. Dr. Pieracci is evaluating the risk/benefit profile of intravenous iron supplementation for this population, following research that suggests this treatment is effective in anemic patients with other inflammatory conditions.
Dr. Pieracci and his team have enrolled more than 70 patients thus far at the lead site and received approval for two satellite study centers: Harborview and the Univ. of Pennsylvania. Enrollment at the satellite centers, as well as establishment of a fourth site, Univ. of Pittsburgh, is ongoing. Dr. Pieracci will present "Baseline Iron Markers in Critically Ill Trauma Patients," based on results from the first 40 subjects, at the Academic Surgical Congress Annual Meeting in Las Vegas in February, 2012. Comparative Effectiveness of Clinical Care Processes in Resuscitation and Management of Moderate to Severe Traumatic Injuries

With recent reports that some trauma centers have higher rates of mortality than others, Dr. Shafi believes that specific processes that improve patient outcomes, as well as those that raise the costs of care without improving outcomes, can be identified. This multicenter study includes four Level I Trauma Centers and measures initial assessment, resuscitation, hemorrhage control, operative care, critical care, rehabilitation, and patient outcomes of severely injured patients. Dr. Shafi and his team will calculate the potential for improvement to patient outcomes with identification and increased use of the best processes of care.
Dr. Shafi reports that data collection should be completed by March 2012, and that he will be presenting at NTI's Advances in Trauma Conference in May. Read More.
